I hereby request to become a student. I understand that there is a cost for the Courses to be mailed and an
additional non-refundable application fee of $15.  I am in agreement with the Doctrinal Statement; and promise to
abide by the Conduct Standards. I have thoroughly read through the catalogue and understand that OFMBI is a
Satellite Teaching and Testing Center of ICW Bible College and Seminary.  All Diplomas / Certificates /
Degrees are granted through ICW who has Religious-Only Degree granting authority by their respective states
of incorporation. I certify that all the information on this application is true and accurate to the best of my
knowledge.   You agree to the terms and conditions listed on this application and catalogue by your E-Signature.
REFUND POLICY
A student may withdraw from a degree program at any time. However, OFMBI requests that the intent be
confirmed in writing. The student will receive a full refund of any payment made within seventy-two (72) hours
of enrollment date except the application fee. If cancellation occurs after seventy-two (72) hours enrollment date
no refund will be rendered.

  • The Educational Programs offered are accredited by a private accrediting organization not associated
    with the US Department of Education
  • All tuition, fees, and lessons must be up to-date in order to receive your diploma/degree/certificate
  • You will adhere to the standards of OFMBI and ICW Bible College and Seminary as a student
If you are planning on using a degree from us for use at another education institution, or for licensing or
credentialing, or for advancement at your place of employment, we advise you to check with that school,
them. This should be done before enrolling in any degree program through OFMBI.
PRIVACY POLICY
Student records, other than name, date of enrollment, degree program, and date of graduation, will not be
released to a third party without the students written permission. This also includes any government
officials or organizations, unless they have a warrant to do so.
